Poor posture is one of the most common and underappreciated causes of back pain, neck pain, headaches and a wide range of musculoskeletal problems. At Sloane Square Chiropractors in Chelsea, postural assessment forms a fundamental part of every initial consultation — because understanding how you hold and move your body is essential to identifying the root cause of your pain.
Why Does Posture Matter?
The human spine is designed to maintain a series of natural curves — a forward curve in the neck (cervical lordosis), a backward curve in the mid back (thoracic kyphosis) and a forward curve in the lower back (lumbar lordosis). These curves distribute the mechanical loads of daily life optimally across the spine.
When these curves are altered by habitual poor posture, the mechanical loading of the spinal joints, discs and muscles changes significantly. Over time, this leads to accelerated wear and tear, muscle imbalances, joint dysfunction and pain.
Common Postural Problems We Assess and Treat
Forward Head Posture
The most common postural problem in the modern age — driven by prolonged screen use, desk work and smartphone use. For every inch the head moves forward from its neutral position, the effective weight on the cervical spine increases by approximately 10 pounds. This creates cumulative stress on the neck joints, muscles and discs.
Upper Crossed Syndrome
A pattern of muscle imbalance characterised by tight chest and neck muscles combined with weak deep neck flexors and lower trapezius. It produces a characteristic rounded shoulder and forward head posture and is a common cause of neck, shoulder and upper back pain.
Lower Crossed Syndrome
Tight hip flexors and lower back extensors combined with weak abdominals and gluteal muscles produce an excessive lumbar lordosis (arched lower back) and anterior pelvic tilt. This pattern is common in office workers and is a significant cause of lower back pain.
Scoliosis
Lateral curvature of the spine — either structural or functional. We assess and manage scoliosis conservatively, providing treatment and advice to minimise progression and manage symptoms.
Pelvic Tilt and Leg Length Discrepancy
Asymmetries in pelvic position and leg length alter the mechanics of the entire spine and lower limb. We assess for these factors and address them through manipulation, soft tissue therapy and corrective exercises.
What Does a Postural Assessment Involve?
- Static postural analysis — assessment of spinal curves, pelvic alignment and shoulder symmetry from front, back and side
- Dynamic movement assessment — observation of gait, functional movements and any compensatory patterns
- Muscle length and strength testing — identifying specific muscle imbalances contributing to postural dysfunction
- Orthopaedic and neurological testing where indicated
- Photographic postural analysis where helpful
Treatment and Postural Correction
Postural correction is a gradual process. Our treatment approach combines:
- Chiropractic manipulation to restore normal joint mobility
- Soft tissue therapy to release tight muscles
- Specific corrective exercises to strengthen weak muscles
- Detailed ergonomic advice for workstation setup, sleep position and daily habits
- Progressive rehabilitation to maintain improvements long-term

Frequently Asked Questions — Postural Assessment
Can posture really cause that much pain?
Yes — the research is clear that sustained poor posture is a significant cause of musculoskeletal pain. Many patients who have suffered years of back, neck or shoulder pain find that addressing the underlying postural causes produces lasting relief where other treatments have only provided temporary benefit.
How long does it take to correct posture?
This depends on how long poor posture has been present and the degree of muscle imbalance and joint dysfunction involved. Most patients begin to see meaningful improvements within 4-8 weeks of consistent treatment and exercise. Long-term postural change requires ongoing commitment to the exercise programme and lifestyle modifications we recommend.
