Sciatica is one of the most debilitating conditions we treat at Sloane Square Chiropractors. The intense radiating pain, numbness and tingling that travels from the lower back through the buttock and down the leg can make everyday activities extremely difficult. The good news is that chiropractic treatment is highly effective for sciatica — and in most cases, surgery can be avoided entirely.
We have been treating sciatica patients across Chelsea, Knightsbridge and Belgravia from our clinic at 9a Wilbraham Place since 2001.
What Is Sciatica?
Sciatica is not a diagnosis in itself but a description of symptoms caused by irritation or compression of the sciatic nerve — the longest nerve in the body, running from the lower spine through the buttock and down each leg. When this nerve is compressed or inflamed, it produces characteristic symptoms that can range from a mild ache to severe, incapacitating pain.
What Causes Sciatica?
Disc Protrusion or Herniation
The most common cause of sciatica. When a disc bulges or herniates, the protruding material can press directly on the sciatic nerve root, causing immediate and often severe symptoms.
Spinal Stenosis
Narrowing of the spinal canal can compress the nerve roots as they exit the spine, particularly in older patients. Chiropractic treatment alongside spinal traction can help manage symptoms effectively.
Piriformis Syndrome
The piriformis muscle in the buttock sits directly over the sciatic nerve. When it becomes tight or inflamed — often due to prolonged sitting or sports activities — it can compress the nerve, mimicking classic sciatica symptoms.
Facet Joint Irritation
Irritated or inflamed facet joints can refer pain in a pattern similar to sciatica. Accurate diagnosis is essential to distinguish this from true nerve root compression.
Spondylolisthesis
When one vertebra slips forward over another, it can narrow the space available for the nerve roots, causing sciatic symptoms.
Symptoms of Sciatica
- Shooting, burning or stabbing pain from the lower back into the buttock and leg
- Pain typically affecting one side only
- Numbness or tingling in the leg, calf or foot
- Muscle weakness in the affected leg
- Pain that worsens with sitting, coughing or sneezing
- Difficulty walking or standing for prolonged periods
How We Treat Sciatica
Our treatment approach for sciatica is evidence-based and tailored to the underlying cause of your nerve compression. A combination of the following techniques is typically used:
Spinal Manipulation
Targeted chiropractic adjustments to the lumbar spine restore normal joint mechanics, reduce nerve irritation and create space for the compressed nerve to recover. A 2020 review in the Journal of Manipulative and Physiological Therapeutics confirmed the effectiveness of spinal manipulation for lumbar radiculopathy.
Spinal Traction
Mechanical decompression of the lumbar spine creates negative pressure within the disc, encouraging the protruding material to retract and relieving pressure on the sciatic nerve. Particularly effective for disc-related sciatica.
Soft Tissue Therapy
Targeted treatment of the piriformis and surrounding musculature to relieve compression of the sciatic nerve in the buttock region.
High Pressure Cryotherapy
Applied to the affected area to reduce inflammation and nerve irritation, particularly in the acute phase.
Rehabilitation Exercises
Specific exercises to stabilise the lumbar spine, strengthen the core and reduce the risk of recurrence.

Frequently Asked Questions — Sciatica
How long does sciatica take to resolve?
Most acute sciatica episodes resolve within 6-12 weeks with appropriate treatment. Chronic sciatica may take longer. Early treatment is important — the sooner you seek help, the faster your recovery is likely to be.
Can chiropractic treatment cure sciatica?
Chiropractic treatment is highly effective for most causes of sciatica. Many patients experience significant relief within the first few sessions. In cases of severe disc herniation or spinal stenosis, we will refer you to the appropriate specialist where surgery may be indicated.
Should I rest or stay active with sciatica?
Current clinical evidence strongly supports staying as active as possible with sciatica. Prolonged bed rest is counterproductive. Your chiropractor will advise you on appropriate activity levels and any movements to avoid during your recovery.
